Reporting adverse reactions
An adverse reaction to a drug may be reported by a patient, their legal guardian or a healthcare professional.
An adverse reaction may be reported:
1. By sending a report to the address

4. You can also report an adverse reaction by contacting the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products
To report an adverse reaction, the following information must be provided:
- Patient identification data: initials, age/date of birth, gender
- Identification data of the reporting person: first name, last name, e-mail address or contact telephone number or correspondence address
- Details of the drug suspected of causing the adverse reaction: name, dose, duration of use, batch number, if known
- Description of the adverse reaction or other information on the safety of the drug
